    • Dates: ASAP-Ongoing
    • Schedule: Monday – Friday, 7:00am-5:30pm – Cath Lab at the Hospital except for: Every Wednesday 8:00am -5:00pm – In- Office Clinic, Every 2nd Friday of the month 8:00am -5:00pm – Telemedicine Clinic, Every 4th Friday of the month 8:00am -5:00pm – In-Office Clinic (First Patient at 9:00am)
    • Patients per day: Clinic 20-22 patients
    • Hospital Bed size: 268
    • Department Description: Cath Lab will be hospital consults 3-4 per day, plus EP cases 4-6 a day depending on case length.
    • cardiac electrophysiology; specifically, heart rhythm disturbances
    • EMR: Cerner
    • Certifications: Board Certification: BC or BE required, License Required: AZ Licensed, will look at IMLC if LOQ in hand, DEA, ACLS
    • Credentialing: 30 days
    • Minimum Experience Required: 1 year
